Individuals who believe they meet NDIS eligibility criteria can obtain an Access Request Form (ARF) and Supporting Evidence Form (SEF). Work collaboratively with your GP, allied health professionals, and medical specialists to gather and prepare all required documentation.

Eligible individuals can apply through several methods:

  • Call 1800 800 110 to submit a verbal access request directly, or
  • Download and complete the ARF digitally, then email it to the NDIA with all supporting documentation, or
  • Download the ARF, print the completed form, and post it to the NDIA with supporting materials, or
  • Email NAT@ndis.gov.au requesting an ARF be mailed to your address, or
  • Contact your regional NDIA office directly and request an ARF be posted to you.

Once you've submitted all required information, the NDIA will evaluate your eligibility for NDIS services and supports.

You'll receive written notification regarding their decision through an official 'access decision' letter.

If Your Application Is Approved

When deemed eligible for NDIS support, you'll be contacted to schedule a planning meeting where you'll discuss your specific support requirements and funding needs.

If Your Application Is Declined

Should you not meet NDIS eligibility criteria, you can still receive valuable information and assistance from an NDIS Local Area Coordinator (LAC) who can connect you with community-based supports and services.

If you disagree with the NDIA's access decision, you have the right to request a review through the Administrative Appeals Tribunal (AAT).

The NDIS provides a comprehensive preparation checklist to help you get ready for your planning meeting, which you can access through their official resources.

The NDIA strongly recommends completing Booklet 2 - Planning. You can work through this independently or seek assistance, then bring your completed booklet to your planning meeting.

Remember to gather and bring all relevant medical reports, assessments, and professional evaluations. These documents help the NDIS understand your specific support requirements.

Based on your individual circumstances and support needs, various professionals can assist you throughout the planning process. Research who can best help you develop your plan.

Participating in your planning meeting—whether conducted in person or via telephone—is essential for the NDIS to collect comprehensive information about your needs, enabling collaborative development of your optimal support plan. During this important meeting, you'll discuss your personal goals, daily activities, and objectives you wish to achieve.

Your planning meeting also provides an opportunity to request Support Coordination funding. Support Coordinators help you develop the skills necessary to understand, implement, and effectively utilize your NDIS plan. Nova Care employs experienced and dedicated Support Coordinators who can assist you if Support Coordination funding becomes part of your approved NDIS Plan.

Following plan approval, you'll receive your NDIS plan through multiple channels: delivered in person, mailed to your address, and accessible through the myplace participant portal.

During your planning conversation, you can request your plan be provided in various accessible formats including Braille, electronic text (CD format), large print, audio (CD format), or translated into your preferred language.
